Once a device starts having trouble, it tends to cause trouble for all of the 
devices connected to the same bus.  The messages that you are seeing suggest 
a hardware problem and a problem with the hard drive or CD drive is more 
likely than a problem with the controller.

I suggest rebooting without your CD drive first to see if the problem lies 
there.  If you continue to get similar log entries without your CD drive, 
then it's likely that your hard drive is failing.  If you happen to have 
another controller card lying around, you could swap controller cards to see 
if you continue getting the same errors with the same disk and a different 
controller.  If you don't have the hardware to try this, consider replacing 
the hard drive first and then, if that doesn't solve the problem, the 
controller.
